Endometriosis has a cure with surgery and drugs to inhibit hormones

Endometriosis has a cure with surgery and drugs to inhibit hormones

For this column, we have chosen to talk about the treatment of endometriosis, which is the presence of endometrium - lining tissue of the uterine cavity - in other parts of the body, such as the pelvic region, ovaries, In general, the treatment must combine the surgery with clinical treatment for better control, since the recurrence is very frequent.

The endometrium grows and flakes in the form of menstruation every cycle as result of female hormonal changes. In the foci of endometriosis this cyclicity also occurs, but the desquamation is retained, causing inflammatory reaction, fibrosis and pain. This is a very frequent problem with diverse, non-specific symptoms, sometimes without correlation with the volume of the disease and may be asymptomatic.

Therefore, it is difficult to evaluate its real incidence in the population, besides performing the early diagnosis. We suspect endometriosis when, among other factors, there is important, growing and incapacitating menstrual pain, mainly that it persists after menstruation. Its frequency is very large in patients with infertility, reaching up to 50% of cases compared to 6 to 7% without infertility, demonstrating that there is a great link between infertility and endometriosis.

Hormones are fundamental for maintenance and growth of endometriosis, so the occurrence is greater between 25 and 35 years. Women who menstruate less, such as those with multiple births, longer breastfeeding, later menarche and early menopause, have a protective effect against endometriosis.

From the symptoms, clinical diagnosis and imaging, we staged the disease to evaluate the best treatment and follow-up. As we have seen, the endometrium depends on hormones to grow and flake, so we can use drugs that atrophy the endometrium and the foci of endometriosis, such as pills, progesterones, and the analogues that induce a woman to a temporary menopause. Surgeries are necessary for diagnosis and especially in cases of advanced diseases for removal of the implants.

Endometriosis often involves other organs of the pelvis, and some of them need to be removed, such as the intestine, bladder, etc. These are called deep endometriosis. In these cases the surgical team should be prepared for this approach by laparoscopy along with the gastric surgeon treating all kinds of complications that may occur during the surgery. In addition to the already established laparoscopy, robotic surgery allows the trained physician a greater facility in operating the complex cases. In general, the treatment needs to combine the surgery with clinical treatment for better control, since the relapse is very frequent, proportional to the extent of the disease and the success of the removal of the implants in the surgery. Endometriosis is a chronic disease of many faces, undetermined, prolonged, sometimes disabling symptoms, diverse and multidisciplinary treatments. Therefore, patient awareness and follow-up by a medical team specializing in all types of approaches are critical to a better quality of life for women with endometriosis.
