The spots appear in exposed areas, are extensive and without delimitation. Melasma can be divided into malar (in the cheekbones), centrofacial and mandibular, depending on the region in which it appears. Today there is a tendency to use herbal medicines and vitamins for the treatment of melasma. A novelty is the Polypodium leucotomos, which is an active of a Costa Rican plant very effective in protecting the skin from ultraviolet rays. This is an anti-inflammatory agent, which protects against cellular damage and has calming properties.

Another option is tranexamic acid, a substance that blocks the conversion of plasminogen to plasmin, a substance that causes hyperpigmentation of melasma, inhibiting its formation . The use of tranexamic acid can be oral, which acts on plasmin, avoiding the formation of inflammatory agents that cause negative stimulus to the melanocyte. This substance is also used in intradermal injections and creams in the concentration of 3 to 5%.

The Q-Switch laser releases much lower energy, up to 3mJ. When we use other lasers or even pulsed light, usually the energy is much higher, around 30-100mJ, releasing much more heat. This laser emits very little heat and does not stimulate the generation of melanin. In addition, the pulse is much faster. While the other lasers emit in milliseconds, the Q-Switch laser emits in nanoseconds.

The combination of low energies and very fast pulse causes the melanin pigment to be destroyed by movement and does not release significant heat. In this way, the rebound effect does not occur. The protocols with the melasma treatment laser have shown that with 12 to 15 weekly sessions a bleaching effect is achieved.

Even with so many new features, the use of the filter remains important, but we keep the skin clearer longer and without the danger of the rebound effect. The cure of melasma does not yet exist, but the results and the possibility of maintenance bring hope to the patients.
