Psoriatic arthritis increases chances of having metabolic and emotional diseases

Normally this disease must be closely monitored by rheumatologists and dermatologists, each of them taking care of one aspect. However, you need to worry more than your skin and joints. "It is accepted in the medical community that this disease can also cause a greater predisposition to some other diseases," says rheumatologist Rubens Bonfiglioli, professor of rheumatology at the Faculty of Medicine of PUC Campinas, vice president of the Paulista Society of Rheumatology and member of the Commission of the Spondylarthritis of the Brazilian Society of Rheumatology.

The following are the diseases most related to the problem and the care they ask for:

Metabolism problems

People with psoriatic arthritis tend to present more metabolic diseases such as obesity, diabetes, hypertension, high cholesterol and triglycerides and even increased rates of uric acid. There are several theories on the relationship between these diseases. "The most commonly accepted hypothesis is that psoriatic arthritis causes inflammation in the body. Since obesity and other diseases are also of inflammatory origin, it is likely that the relationship will come from this process," says Bonfiglioli

In addition, obesity alone is already related to an increased risk of metabolic diseases. And people with psoriatic arthritis have a number of factors that lead to weight gain. Many people with this condition end up exercising less, both due to problems in the joints and difficulties in going outside and exposing their injuries. Drugs can also be a problem, says the rheumatologist Cristiano Barbosa Campanholo, a member of the Brazilian Society of Rheumatology and a member of the Research and Evaluation Group of Psoriasis and Psoriatic Arthritis (GRAPPA - Group for Research and Assessment of Psoriasis and Psoriatic Arthritis, in free translation group Psoriatic Arthritis and Psoriatic Arthritis).

Other mechanisms are also involved in the onset of these related diseases. One example is high cholesterol: "some proteins interfere with the relaxation of blood vessels, thus causing greater stress within the vessel and, consequently, a greater possibility of an increase in cholesterol", explains Campanholo

The problem is that these metabolic diseases cause various problems in the body and also increase the risks of cardiovascular conditions such as heart attack and stroke. And even without fully established processes, it is a fact that people suffering from psoriatic arthritis need to worry about these diseases, since it has 30 to 40% more chances to present them. Therefore, lifestyle changes are essential: having a proper and healthy diet, not smoking, reducing alcohol intake, practicing physical activity three to four times a week.

Depression and anxiety

Psoriatic arthritis but the consequences are also observed in the psychological and emotional health of the patient. As skin lesions are often large and well-apparent, they need to deal with problems such as prejudice and low self-esteem. It is very common for the patient with these cadres to suffer a social stigma. In addition, chronic pain and discomfort increase the incidence of depression and anxiety in these people.

And these emotional factors can even cause a vicious circle. "There is a tendency to consider depression and other psychological changes as triggering factors of cutaneous psoriasis, further worsening the appearance of lesions," Bonfiglioli says. Therefore, often monitoring with psychologist or psychiatrist may be important for anyone who has psoriatic arthritis. The patient also needs to accept the lesions and not consider them an impediment to getting out or leading a normal life, after all they are not contagious and following the appropriate treatment, they soon reduce.

Eye diseases

Campanholo highlights another problem of common health in those who have psoriatic arthritis: eye disorders. "It is important to follow up with an ophthalmologist, because psoriatic arthritis can reach any part of the eye. It can happen from a redness and pain in the eyes, similar to a conjunctivitis, to a difficulty to see, with temporary loss of vision ", alert. Therefore, it is important to always be aware of eye health when you have this picture.

Multidisciplinary treatment is fundamental

With the risk of so many related diseases, multidisciplinary treatment becomes fundamental. "It is always important for the patient to go through a cardiologist, an endocrinologist, an ophthalmologist and even a psychiatrist, according to the diseases that the rheumatologist perceives," explains Bonfiglioli.

It is very common for the patient to arrive with one of these related diseases already installed, or hypertension. "The rheumatologist is first and foremost a general practitioner, able to recognize these problems and make the correct referral," says the specialist.
