Silicone: bigger and bigger prostheses bring risks

Big, firm breasts: the silicone implant is tempting for most women. However, many are the care that this surgery demands. According to the Brazilian Association of Plastic Surgery, 21% of the approximately 460,000 cosmetic surgeries performed last year were breast augmentation. Recently, the Brazilian with the largest breasts in the country, Sheyla Almeida Hershey (3.5 liters in each), suffered complications with an infection caused in the most recent of the 30 surgeries she has done throughout life to increase prostheses.

Sheyla's oversized measurements are impressive, but the preference for heavier implants is a trend among women. A survey of Ibope performed with doctors of the Brazilian Society of Plastic Surgery confirms what we can observe there: the size of the prosthesis chosen by women is increasing. According to the survey, the most commonly used silicone prosthesis is 300 ml, at the beginning of the decade, the most popular was 250 ml and in the 1990s, it was 190 ml.

Size is document?

However, erratic prosthesis size is a serious health issue - not just aesthetics. "As important as solving this aesthetic disharmony is adjusting the size of the implant to the body type of each woman. A large prosthesis looks good for a tall woman or who has wide hips, and in the small ones it may appear that the woman is overweight

"It is very common to arrive for the consultation wishing a volume similar to that of the friend, who was good, but will not always fit well in another body," says Rubens Penteado, director of the Center for Integrated Medicine (SP). the plastic surgeon Luciana Pepino. Before choosing the prosthesis and deciding to increase many numbers of the bra at once, there are two main issues to be observed: the new forms should allow you to feel comfortable at all times, whether at a party, at the beach or at work.

The second involves imagining with the prosthesis in a more distant future. "The younger patients, in particular, are those who opt for larger implants to achieve a more generous neckline. What many forget is that fashion is fleeting, but aging is not," he warned. "We must think, for example, if 50 years old, we will still like this look," he says.

Risks

"Like any surgery, the silicone implant has possible risks and complications, which, be explained to the patient, "says plastic surgeon Ubirajara Freitas Guazzelli. Risks of hematoma (bleeding), infection, capsule contracture, prosthesis displacement, hypertrophic or keloid scars, loss of sensation, anaphylactic shock, and death are rare, but they do exist. In addition, there is the problem of the appearance of stretch marks.

The greater the volume of the prosthesis, the greater the likelihood of the lines appearing, since the fibers of the skin will undergo a tearing. The weight of the breast will also increase, so overloading of the skeleton may occur, which will cause forward projection of the trunk, causing postural problems, back pain and even a deviated spine.

Silicone is not indicated for patients who have some immune disease, more for the surgical consequences it can bring than for the material itself. There is also no problem in putting silicone implants in the breasts and getting pregnant. Silicone does not prevent or disrupt breastfeeding.

In addition to silicone

If sagging is the biggest problem for you, the silicone may not give you the expected result. "Prosthetics placed on flaccid breasts have a high chance of turning large, flaccid breasts after a few months because the function of the prosthesis is to fill an empty tissue and if there is flaccidity it is necessary for the woman to first do a plastic to lift flaccid breasts, whose function is to remove excess skin to promote breast lift, "says plastic surgeon Luciana Pepino. In this case, if she still wishes, the prosthesis can be placed in the same surgical act or later, at the discretion of the doctor and the patient. Another common breast plastic surgery is the reduction mastoplasty, indicated for the reduction of the breasts. Usually this surgery, when indicated, in addition to improving the aesthetic aspect of the breast, also prevents problems caused by very large breasts.

Firmness in the gym

For those who are not afraid to face the workout, there are bodybuilding exercises that avoid sagging breasts. Personal trainer Ivaldo Larentis explains that it is not exactly the stiffening breasts, but the muscles that surround them. "Breasts are fat and there is no way to tighten body fat, but bodybuilding exercises can easily work the pectoral muscles, while keeping the skin firm," he says. "For the whole thing to get even better, the ideal is to combine aerobic exercises, which do not let the breast fat increase, leaving them firm longer."

Otoplasty: plastic surgery corrects fanned ears

The term otoplasty refers to plastic surgery of the ears, and may correspond to several techniques that may or may not be associated, depending on the problem being treated. In general, the term is used to indicate the protrusion of prominent (flap) ears, but other problems such as trauma sequelae, congenital absence of ears and constricted ears are also treated with otoplasty techniques.
